HOW TO USE THESE FORMS

 

Each of the microsoft word files have been formatted for use of “check the box” forms and text input forms.  If you want to modify the forms, go to the “Tools” on the tool bar and click on the “unprotect” bar.  In the alternative, you can bring up the “Forms Toolbar” and click on the small lock on the toolbar.  Each form can be printed as either (1) adobeacrobat.pdf, with the assumption that you will print the form and fill-in the blanks, (2) microsoft word format, again formatted to be printed and then fill-in the blanks, and (3) microsoft word, formatted to be downloaded and completed on your computer and then printed.  The forms are presented in several sub-groups as follows:

 

  1. U - Pro-Se Generic Uniform Forms.  This file includes uniform forms that can be used for most guardianship/conservatorship pleadings.  While it is anticipated that pro-se litigants will use the forms, professionals will also use these forms.  Also, forms such as the acceptance of appointment, the inventory or annual account are all uniform forms that are found in this file.  These forms contain a large amount of “check the box” i.e.,  options.  If you want a portion of the form to apply you should check the box.

 

  1. JT - Joint Guardian and Conservator Forms.  These forms are for pleadings for a guardian/conservator appointment and subsequent pleadings.  These forms assume you know what you are doing (as opposed to the pro-se forms).  If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. G - Guardian only forms.  These are for pleadings related only to guardians. If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. C - Conservator only forms.  These are for pleadings related only to conservators. If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. GM - Guardian of a minor forms. These are for pleadings related only to guardians of a minor. If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. GA - Appointment by a parent or spouse; pre-approved appoint of guardian.  Under the new act, you may petition the court to confirm your choice of a guardian before incapacity disables the acting guardian or before the ward becomes incapacitated.  If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. CM - Conservator of a minor forms. These are for pleadings related only to conservators of a minor. If you are missing a form, consult the Pro-Se Uniform Forms file.

 

  1. JTM – Joint petition for guardian and conservator of a minor forms.  These pleadings are for the appointment of a guardian and conservator for minors only.  If you are missing a form, consult the Pro-Se Uniform Forms file.

 

    9.    UM - Pro - Se Generic Forms for Guardian/Conservator of a Minor.  These forms are for the appointment of a guardian for a minor

           or for appointment of a conservator for a minor's estate, or for both a guardian and conservator appointment for a minor.

 

    10.   MISC – Miscellaneous forms.  These forms include generic forms to petition for new letters, to petition for proceeding in forma

            pauperis, to apply with the DHS for a criminal background check; to file a notice of lis pendens, and to file a certificate of   

            representation.
